Anticoagulation Management
Anticoagulant medications are prescribed to patients who experience certain heart conditions or have had a heart attack. These medications help to prevent blood clotting and strokes, and to minimize risk for bleeding complications.
Our care team provides personalized care for patients taking blood-thinning or anticoagulation medications. Each patient’s therapy is closely managed by our experienced team of clinical pharmacists and supervising physicians.
Other conditions for which anticoagulation medications may be prescribed are pulmonary embolism, deep vein thrombosis (DVT) or atrial fibrillation.
