Many Coquille Valley Hospital patients take advantage of our home health care programs as a way to obtain quality health care in familiar and comforting home surroundings. Our home care teams not only provide care: they teach families what they need to know to help them care for their family member, and patients what they need to know to help themselves.

Who qualifies for Home Health Services? How do I get referred?

Anyone who has health care needs and is under a physician's care qualifies for Home Health Services. All care given by the home health team must be ordered by a physician, and often it is the patient's physician or hospital staff who makes the referral to Home Health Services as part of the patient's discharge plan. Other individuals - such as friends and relatives of the patient, or community agencies aware of the patient's condition - may also make referrals. Home Health then contacts the physician for orders to visit and to evaluate the need for care.

What services are available?

The Home Health team designs an individualized plan of care based on each patient's changing needs. These may include (but are not limited to) the following services and treatments:

Nursing Care

  • Teaching the patient and family nursing skills to foster independence
  • Monitoring cardiovascular and pulmonary condition
  • Drawing blood, giving injections and IV therapy
  • Wound care and dressing changes
  • Diet, medication and exercise instruction for diabetics and others
  • Other specialized care requiring the skills of a registered nurse

Home Health Aide

  • Eligible patients may receive assistance with personal care such as bathing, hair care, shaving, and light home care tasks.

Physical Therapy

  • Evaluating physical abilities and therapy needs
  • Teaching therapeutic exercises designed to help ease pain, correct deformity, increase strength and mobility and improve general health
  • Teaching patients with physical impairments to walk safely

Speech Therapy

  • Evaluating speech and swallowing related problems and determining therapy needs
  • Retraining patients with loss of language use and comprehension following stroke and head injury
  • Follow-up and training for laryngectomees
  • Swallowing therapy

Does Home Health supply medical supplies?

The Home Health team can assist in arranging for medical supplies and equipment for patient use.

Is there an emergency lifeline program?

Lifeline is a hospital-based personal emergency response system. It is designed especially for elderly and disabled persons living alone. Lifeline provides immediate response in emergencies at the simple touch of a button. New Lifeline units are installed subject to availability. Contact Coquille Valley Hospital for more details.

Can I get assistance through Home Health Care at any time?

Home Health Services are provided on a part-time, intermittent basis. How often we visit depends upon your condition and your doctor's orders. We make every effort to help you become independent by teaching you and your family the skills you need to assume your own care.

Home Health Services' office hours are Monday through Friday, 9:00 AM to 5:00 PM. Weekend and holiday visits by Home Health teams are limited to special medical and nursing needs.

As always, patients are encouraged to keep regular appointments with their physicians.

Does my insurance cover Home Health Care?

Home Health Services is a Medicare-reimbursed program and is typically covered under other medical insurance policies.
